How long will you be on goserelin?
The length of goserelin treatment depends on why it was started. After surgery it is often given for up to five years alongside a hormone tablet. To protect the ovaries it is given only during chemotherapy. For advanced disease it continues while it helps. This page explains each pattern, when length is reviewed and what happens when goserelin ends.

The short answer
How long goserelin is usually given
How long you stay on goserelin (Zoladex) depends mainly on why it was started. For premenopausal women with hormone receptor positive early breast cancer who are at higher risk of the cancer returning, goserelin is usually given alongside tamoxifen or an aromatase inhibitor for up to five years. That is the length used in the large trials that showed a benefit, and it is what most guidelines describe. Some teams use a shorter course, such as two or three years, particularly when side effects are hard to live with or the benefit is expected to be smaller, and that is a reasonable conversation to have. When goserelin is given only to help protect the ovaries during chemotherapy, it is started shortly before chemotherapy and stopped when chemotherapy finishes, so the course lasts a matter of months. For advanced breast cancer, goserelin is usually continued for as long as the hormone treatment it supports is controlling the cancer and you are coping with it, which may be many months or years. If the partner medicine changes, goserelin often continues so the ovaries stay switched off. As you approach the usual age of natural menopause, your team may review whether it is still needed. Whatever the plan, the length should be agreed with your oncologist and revisited over time.

Goserelin is usually given for the same period as the hormone tablet it is paired with, or less.
Ovary protection: during chemotherapy only
It starts before chemotherapy and ends when chemotherapy ends.
Advanced disease: as long as it helps

What is still uncertain about treatment length
The ideal length of ovarian suppression is not settled for everyone, and it is fair to say so.
Trials tested set lengths
Most of the evidence comes from studies that gave ovarian suppression for five years, while others used two. Direct comparisons of different lengths are limited.
Benefit and burden both build up
Longer treatment may add protection for higher-risk women, but side effects also accumulate over the years.
Stopping early is common
Many women find the years on ovarian suppression hard. Stopping early should be a shared decision rather than a quiet one.
What this page cannot tell you
It cannot tell you the right length of treatment for you. That depends on your risk, how you are coping and your plans, which your oncologist can weigh with you.
After goserelin ends
What often comes next
Stopping goserelin does not always mean stopping all hormone treatment.
If you are still premenopausal
Your ovaries may start working again. If further hormone treatment is advised, tamoxifen is usually used, because aromatase inhibitors do not work well while the ovaries are active.
If you have reached menopause
Your team may use blood tests over time to check whether your ovaries have stopped for good. If they have, an aromatase inhibitor may be considered for extended treatment.
Bone and wellbeing checks
A repeat bone scan and a review of how you are feeling are sensible as treatment ends.

Wanting to stop is understandable, and there are usually options to discuss.
Talk first
Your team can explain how much extra benefit you might be giving up, and whether side effects can be eased with other help.
Consider alternatives
Switching from an aromatase inhibitor to tamoxifen, shortening the planned course, or choosing surgery to remove the ovaries are all possible paths for some women.
Pregnancy wishes
Some women pause hormone treatment to try for a baby after discussion with their oncologist, then restart.

Common questions about the length of goserelin treatment
Why five years?
Five years is the length tested in the main trials of ovarian suppression for early breast cancer, and it matches the usual length of hormone tablets. Because it was the length studied, it is the standard most guidelines refer to. Shorter courses are sometimes used when the balance of benefit and side effects favours it.
Can I stop goserelin at two years?
Some studies used two years of ovarian suppression and found a benefit, so a shorter course may be reasonable for some women. It depends on your risk and how you are coping. Discuss it with your oncologist rather than stopping on your own.
Will I need goserelin after I turn fifty?
As you reach the usual age of natural menopause, your team may review whether goserelin is still needed. Because goserelin masks whether your ovaries would have stopped on their own, this can take careful planning and blood tests over time.
What happens to my periods when I stop?
If your ovaries recover, periods usually return within some months of the last implant. For women close to natural menopause, or who had chemotherapy, periods may not return. Use contraception if you do not want to become pregnant.
How long does goserelin keep working after the last implant?
Each implant lasts about four weeks. After that, hormone levels begin to recover over the following weeks to months. The pace varies with age and past treatment, so do not assume you cannot become pregnant soon after stopping.
Is the course longer for advanced breast cancer?
Usually yes. There is no fixed end date. Goserelin generally continues while hormone treatment is controlling the cancer, and may continue with a new partner medicine if the cancer grows. Your team will review it at each change in treatment.
Can I switch to surgery partway through?
Yes, some women choose keyhole surgery to remove the ovaries instead of continuing monthly implants. It is permanent and means no chance of natural pregnancy afterwards, so it needs careful thought and a conversation with your oncologist and a gynaecologist.
Does a break in treatment reset the five years?
Not usually. Short breaks, for example to try for a pregnancy, are counted differently by different teams, and treatment often resumes to complete the planned course. Ask your oncologist how any break would be handled in your case.
