Taking exemestane: timing, food and daily routine
Exemestane is a once-a-day tablet taken after food, often for several years. The short answer
How should you take exemestane each day?
Exemestane is taken as one tablet once a day, at the dose your doctor prescribes, and it is best taken after a meal. Food helps your body absorb the medicine more consistently, so many women take it straight after breakfast or dinner. Swallow the tablet whole with a glass of water. The exact time of day matters less than keeping to a similar time and always taking it after eating, because a steady routine keeps oestrogen levels reliably low and makes it much easier to remember. Keep the tablets in their original pack, at room temperature, away from heat, damp and direct sunlight, and out of the sight and reach of children. If you forget a dose, take it later the same day if you remember, or skip it if your next dose is nearly due; never take two together. Exemestane can usually be taken alongside calcium and vitamin D, bone-strengthening medicines and most everyday treatments, but check anything new with your pharmacist, and avoid products containing oestrogen, such as hormone replacement therapy. If you are premenopausal, exemestane is only started once your ovaries have been switched off with injections or surgery, and your team may check hormone levels first. For early breast cancer, it is usually taken for several years, so building a simple habit from the start really helps. If side effects make taking the tablet hard, talk to your team rather than stopping.

Being straight with you
What routine can and cannot fix
A good routine solves forgetting, but it does not solve everything that makes long-term tablets hard.
Side effects need their own help
If joint pain, hot flushes or low mood make you want to stop, a reminder app will not fix that. Your team can offer treatments or a different tablet.
Small timing differences are fine
There is no evidence that taking exemestane an hour or two earlier or later on some days makes any difference.
What this page cannot tell you
It cannot replace the instructions for your own situation, especially if you take other medicines such as everolimus or ovarian suppression injections.
Getting started
Before and during your first weeks
A little preparation at the start makes the following years smoother.
Before your first tablet
Your team will confirm your menopausal status and may arrange a bone density scan and blood tests. Tell them about every medicine, vitamin and herbal product you take.
If you are premenopausal
Ovarian suppression injections are usually started first, and exemestane follows once oestrogen is low. Keep every injection appointment, because exemestane alone does not work if the ovaries are active.
The first weeks
Note any side effects in a diary, with when they started and how strong they feel. This helps your team at your first review.
Other tablets
Taking exemestane alongside other medicines
Many women take several medicines. A few simple points keep things safe.
Calcium and vitamin D
These can be taken at the same meal as exemestane, as your team advises.
Bone medicines
Bisphosphonate tablets have strict instructions, often on an empty stomach first thing in the morning, so take exemestane later after a meal.
Everolimus in advanced cancer
If you take everolimus too, follow the timing and food advice given for that combination, and keep up the regular blood tests it needs.
Anything containing oestrogen
Avoid HRT, hormonal contraception and most vaginal oestrogen unless your oncologist has agreed.
Commonly believed
Myths about taking exemestane
Roughly the same time, after food, is enough.
The opposite is true. Food helps absorption, which is why it is taken after a meal.
Clear checks are expected. The tablets guard against cells too small to see, so finish the planned course.
Never share prescription medicines. Her treatment may be quite different from yours.

Common questions about taking exemestane
Is it better to take exemestane in the morning or at night?
Either is fine, as long as you take it after a meal and at a similar time each day. Some women prefer the evening because they eat dinner more reliably. Others prefer the morning so they do not forget when tired. Choose what fits your routine best.
Can I crush or split exemestane tablets?
Exemestane tablets are meant to be swallowed whole. If you have trouble swallowing, talk to your pharmacist before crushing or splitting them, as they can advise whether this is suitable for your brand or suggest other ways to make swallowing easier.
Can I drink alcohol while taking exemestane?
Alcohol does not stop exemestane working, but it can make hot flushes worse, affect sleep and bone health, and is linked to a higher risk of breast cancer. Keeping alcohol to a minimum, or avoiding it, is the healthiest choice during and after treatment.
Can family members handle my tablets?
Yes. Handling intact exemestane tablets is considered low risk. Carers can wash their hands afterwards as a sensible habit. Tablets should be kept in their packaging and out of the reach of children, and anyone who is pregnant should avoid handling broken tablets where possible.
Can I switch brands of exemestane?
Approved generic brands contain the same active medicine, so changing brand is generally fine. The tablets may look different. If you notice any change in how you feel after switching, tell your pharmacist or team. Sticking with one brand where possible makes this easier to track.
What if I vomit shortly after taking my tablet?
Do not take another tablet. Take your next dose at the usual time the following day. If you are being sick often, tell your team, as they can suggest ways to settle your stomach and check whether something else is causing it.
Do I need to stop exemestane before dental work or surgery?
Usually not for routine dental work. For operations, tell the surgical and anaesthetic teams that you take exemestane and ask whether to continue it. If you also take bone-strengthening medicine, mention this to your dentist before any tooth extraction.
How long does a pack last, and how should I store spares?
Pack sizes vary between brands, so check how many tablets yours contains and set a reminder to buy more before it runs out. Keep spare packs sealed, at room temperature, somewhere dry, and check expiry dates from time to time.
