Pertuzumab myths: 'it is just a second Herceptin'

Pertuzumab is often mistaken for a copy of trastuzumab, but it attaches to a different part of HER2 and blocks the protein from pairing with its partners. The two are given together. This page clears up common myths about how pertuzumab works, hair loss, heart worries and whether everyone with HER2-positive cancer needs it.

The short answer

Is pertuzumab just a second Herceptin?

No. Pertuzumab, sold as Perjeta, and trastuzumab, often known by the brand name Herceptin, are both antibodies that attach to HER2, the growth protein found in large amounts on HER2-positive breast cancer cells. But they attach to different parts of that protein and work in complementary ways. HER2 sends growth signals most strongly when it pairs up with partner proteins, especially one called HER3. Pertuzumab sits on the part of HER2 that it uses to pair up, so it blocks those partnerships. Trastuzumab sits on a different part and mainly disrupts signalling from HER2 on its own, while also flagging the cell to the immune system. Using both together is called dual HER2 blockade, and research shows the combination does more than trastuzumab alone in some situations. That is why pertuzumab is always given with trastuzumab, not instead of it. Other common beliefs also deserve a closer look. Pertuzumab is not chemotherapy, and it does not cause hair loss, although the chemotherapy often given with it does. It is not needed by everyone with HER2-positive cancer, since its benefit depends on stage and risk. It does add some side effects, mainly diarrhoea, but heart problems do not appear to be much more common than with trastuzumab alone. Clearing up these myths helps you understand why it is being offered.

What trastuzumab does

It attaches to one part of HER2, reduces growth signals from HER2 and helps immune cells find and attack the cancer cell.

What pertuzumab does

It attaches to the pairing part of HER2 and stops it joining with partners such as HER3, which blocks a strong growth signal.

Both also help mark the cell for the immune system.

Why combine them

Blocking HER2 in two ways shuts down more of the growth signalling than either antibody alone.

How they are given

Usually as separate drips on the same day every three weeks, or as a combined injection under the skin where available.

Often given with

  • Chemotherapy such as docetaxel or paclitaxel
  • Hormone therapy for hormone-positive cancer
  • Regular heart scans

Honest limits of what pertuzumab offers

Correcting myths should not swing too far the other way. Pertuzumab is valuable, but it has limits worth knowing.

The benefit varies a lot

As a first treatment for advanced cancer it helped people live longer. After surgery in early cancer, the added benefit was modest and mostly in higher-risk cancers.

It does add side effects

Diarrhoea is the main extra side effect, especially during chemotherapy, and rash or tiredness can also increase.

It adds cost

For many families in India, the extra expense is significant and fair to discuss.

What this page cannot tell you

It cannot tell you whether pertuzumab will help your cancer. Your oncologist can explain your likely benefit.

Three more misunderstandings

Some beliefs come from confusing pertuzumab with other medicines given at the same time, or from older information.

"Pertuzumab can replace chemotherapy"

In standard treatment, pertuzumab and trastuzumab are given with chemotherapy at first. Studies are exploring whether some people can safely have less chemotherapy, but that is decided case by case.

"If cancer remains after surgery, pertuzumab has failed"

Not necessarily. Some cancers are harder to clear. In that situation, switching to T-DM1 after surgery is often recommended to lower the chance of recurrence.

"Diarrhoea means stopping treatment"

Diarrhoea is common, especially with chemotherapy, but is usually manageable with fluids, anti-diarrhoea medicines and sometimes a short pause.

The heart question

Does adding pertuzumab put more strain on the heart?

This is one of the most common worries. Trastuzumab can weaken heart pumping strength in some people, so it is natural to fear that two HER2 antibodies mean twice the risk.

What studies found

Large studies did not show a major increase in heart problems when pertuzumab was added to trastuzumab. Most heart changes were mild and improved when treatment was paused.

Why scans continue

Heart scans are still done before and during treatment, because a small number of people do develop reduced pumping strength.

Symptoms to report

Tell your team about breathlessness, ankle swelling, a racing heartbeat or needing more pillows to sleep.

Common questions about pertuzumab myths

Why do I need both pertuzumab and trastuzumab?

They block HER2 in different ways. Trastuzumab acts on one part of the protein, and pertuzumab stops HER2 pairing with partner proteins. Together they shut down more growth signalling. In the right situations, research shows this combination works better than trastuzumab with chemotherapy alone.

Is pertuzumab a type of chemotherapy?

No. It is a monoclonal antibody, a targeted treatment that attaches to HER2. It is often given at the same time as chemotherapy, which is why people sometimes confuse the two. Once chemotherapy ends, pertuzumab and trastuzumab may continue on their own.

Will pertuzumab make my hair fall out?

Pertuzumab itself is not known to cause significant hair loss. Hair loss during treatment is usually caused by the chemotherapy given alongside it. Once chemotherapy finishes, hair typically starts to regrow even if pertuzumab and trastuzumab continue.

Is the injection under the skin a different medicine?

Where available, a combined injection contains both pertuzumab and trastuzumab, given under the skin instead of by drip. It contains the same kinds of antibodies with an added ingredient that helps absorption. Your team can explain whether it is an option for you.

Does everyone with HER2-positive cancer get pertuzumab?

No. It is mainly used for larger or node-positive early cancers and as a first treatment for advanced cancer. For small, node-negative cancers, trastuzumab with chemotherapy is often considered enough. The decision depends on your risk and your preferences.

Does pertuzumab cause heart failure?

Serious heart problems are uncommon. Studies did not show a large increase in heart problems when pertuzumab was added to trastuzumab. Heart scans are still done regularly so any fall in pumping strength is found early and treatment can be paused if needed.

Is there a cheaper version of pertuzumab?

Availability of other versions depends on approvals, which change over time. Ask your oncologist and pharmacist what is currently approved in India. Cheaper trastuzumab biosimilars do not replace pertuzumab, because they are a different medicine.

If pertuzumab is stopped early, is treatment wasted?

No. Treatment given still contributes, and trastuzumab can usually continue on its own. Pertuzumab may be stopped for side effects, cost or a change in plan after surgery. Your team will explain what the remaining treatment will look like.
