Digital PET scanners use newer detector technology that produces sharper images with less noise. Time-of-flight is a feature that further improves image quality, particularly for detecting small lesions. Both are meaningful advances, but the clinical impact depends on what your scan is looking for and on your body type. Neither replaces a precise clinical question.

What are digital PET and time-of-flight?

A conventional PET scanner detects signals from a radioactive tracer using photomultiplier tubes. A digital PET scanner replaces these with silicon photomultiplier detectors, which record when and where each signal arrives with greater precision.

Time-of-flight is a technique that measures the tiny difference in arrival time between the two photons released at each tracer decay event. Knowing that difference tells the scanner more precisely where in the body each event occurred, which reduces image noise.

Together, these features produce images with better contrast and less background noise than older equipment. The Society of Nuclear Medicine and Molecular Imaging (SNMMI) recognises that digital detectors with time-of-flight improve signal-to-noise ratios, and that this advantage is most pronounced when lesions are small or tracer uptake is low.

When does the technology difference matter most?

  • Small or low-uptake lesionsDigital PET with time-of-flight is less likely to miss a lesion that a conventional scanner might not show clearly, because lower noise makes faint signals more visible.
  • Larger body sizeTime-of-flight provides a greater image quality benefit in people with a larger body habitus, where photon scatter is higher and noise accumulates more.

Frequently asked questions

Does a digital PET-CT scan find more cancers than a conventional one?

For most standard indications, both give your oncologist the information they need. The difference is most meaningful for small or low-activity lesions, where digital PET with time-of-flight is less likely to return an unclear result. SNMMI guidance acknowledges improved sensitivity for smaller lesions, but this does not mean a well-maintained conventional PET-CT is inadequate for the majority of scans. The clinical question your team is asking — what they are looking for and why — determines how much the scanner generation actually matters for your specific scan.

Does a digital PET-CT scan cost more?

It can, and the difference varies by centre. More recently installed equipment can carry higher running costs, which may be reflected in the scan fee. The difference is not large at every centre, and some protocols run faster on digital equipment, which can partially offset the cost. Ask the imaging centre directly when you book: what is the fee, and is the scanner digital with time-of-flight? That is a routine question and imaging centres expect it.

Should I travel further or pay more to get a digital PET-CT?

For most indications, probably not. If your oncologist has ordered a standard staging or restaging scan, a well-maintained conventional PET-CT at an accredited centre will produce a report your team can act on. The benefit of digital with time-of-flight is real but concentrated in specific situations: small lesion detection, larger patients, and high-precision treatment response assessment. Ask your oncologist whether the specific question your scan is meant to answer is one where the scanner generation makes a meaningful clinical difference.

How do I know which type of scanner I will be scanned on?

Ask directly when you book. Imaging centres know their own equipment and will tell you the scanner model if you ask. Your oncologist or referral centre may also know which scanners a particular imaging facility uses. At CION, PET-CT is coordinated with partner imaging centres rather than done in-house — your care team can tell you which facility is being used for your scan, and you can then ask that facility about their equipment.

Does it matter if my follow-up scan is done on a different scanner from my baseline?

It can matter, yes. When your oncologist is comparing scans over time to measure how well treatment is working, a change in scanner model introduces a variable that can make the comparison harder to interpret. This is worth mentioning to your care team if your follow-up is being arranged at a different facility from your baseline scan. It does not always change the clinical decision, but your oncologist should know so they can take it into account when reading the report.

Is time-of-flight PET available through CION?
