Cashless vs Reimbursement — Which Route Gets Your PET-CT Done Faster?

The two insurance routes for a PET-CT work very differently. Cashless requires your insurer to approve the request before the scan happens. Reimbursement lets you get the scan done immediately and reclaim the cost afterwards. Which one suits you depends on how urgently the scan is needed and whether you can pay upfront.

  • Cashless delays the scan — Your insurer must respond to the pre-authorisation request before imaging can begin, which typically adds one to two working days.
  • Reimbursement is immediate — You pay at the imaging centre on the day and claim afterwards — no waiting for approval, but you need to be able to pay the full cost upfront.
  • Denial does not end your options — If cashless pre-auth is refused, you can pay upfront, get the scan done, and then appeal the denial or file a reimbursement claim.
  • Documents differ by route — Pre-auth needs your referral form before the scan. Reimbursement needs original bills and receipts after it.

Reimbursement gets you into the scanner faster — you pay upfront and claim afterwards. Cashless pre-authorisation requires your insurer to approve the request first, which typically takes one to two working days. If you can pay the scan cost upfront and the scan is urgent, reimbursement is the quicker route.

CION's PET-CT scans in Hyderabad start at Rs 10,499 — among the lowest published prices in the city, with no hidden charges. Indicative price, as of September 2026.

What do you do if cashless pre-authorisation is denied?

Denial is not the end of the process. The first step is to ask your insurer for the reason in writing — you are entitled to this under IRDAI guidelines. The most common reasons are a missing document, a policy exclusion, or a waiting period that has not yet elapsed.

If the denial is for a missing document, your oncologist's team can often send the additional information the same day and ask for an expedited reconsideration. If it is a policy exclusion, ask your oncologist's team whether the wording genuinely applies to your specific cancer type and scan indication.

While the appeal is underway, you do not have to wait. Pay for the scan yourself, proceed with the imaging, and file a reimbursement claim immediately afterwards. Keep the denial letter and all correspondence — these form the core of your claim evidence.

Tell your oncologist's team about the denial. A letter from your oncologist explaining why the scan is clinically necessary often resolves the dispute at first appeal.

Did you know?

Pre-authorisation requests are more likely to be approved when the referral explicitly states the cancer type, the stage or response being assessed, and the guideline recommending the scan. A referral that says only 'PET-CT required' gives the insurer's medical assessor very little to match against a covered condition.

Under IRDAI guidelines, you have the right to a written explanation of any pre-auth denial. That letter tells you exactly what to fix — and whether the fastest resolution is adding a document or switching to reimbursement.

Common questions

Frequently asked questions

Which is faster — cashless or reimbursement for a PET-CT?

Reimbursement gets you scanned faster because there is no waiting for insurer approval. You pay at the imaging centre on the day, the scan proceeds immediately, and you submit your documents and claim afterwards. Cashless pre-authorisation typically adds one to two working days before the scan can be confirmed. If the scan is urgent and you can pay upfront, reimbursement removes that delay. If paying upfront is not possible, your oncologist's team can often request expedited pre-auth for urgent cases.

How early should I apply for cashless pre-authorisation before a PET-CT?

Apply as soon as the scan is decided — ideally three to five working days before the planned date. This allows time for the insurer to respond and for you to provide any additional documents they request. Do not book the scan date first and apply for pre-auth after; most insurers will not approve a cashless claim retrospectively for a scan that has already taken place. If the scan is urgent, ask your oncologist to mark the pre-auth request accordingly — IRDAI guidelines require insurers to process urgent requests within shorter timeframes than routine ones.

Can I switch from cashless to reimbursement if pre-auth approval takes too long?

Yes. If the insurer's response is delayed and the scan cannot wait, pay for the scan yourself and submit a reimbursement claim afterwards. Tell the imaging centre you are switching routes and ask for a full itemised receipt rather than a summary bill. Keep the written pre-auth request you submitted, even if it was never approved — this shows your insurer you attempted cashless first and strengthens your reimbursement claim if they question why you did not use that route.

What happens if my insurer only partially approves the cashless pre-authorisation?

Partial approval means your insurer will cover the scan up to the approved amount and you pay the difference on the day as a co-payment. Ask the imaging centre in advance what that shortfall will be so you are not caught out at the desk. If the approved amount is well below the actual cost, ask your oncologist's team to write to the insurer explaining why the full cost is clinically justified. You can also ask the imaging centre whether a clinically appropriate variant of the scan falls within the approved amount — your oncologist needs to confirm it would still answer the clinical question.

Does health insurance always cover a PET-CT scan?

Not always. Coverage depends on your specific policy: whether cancer investigations are included, whether a waiting period applies, and how the scan is classified. The most common reason for denial is that the referral did not describe the clinical indication specifically enough for the insurer's assessor to match it to a covered condition. Ask your oncologist to write the referral with the cancer type, the clinical question the scan will answer, and, where applicable, a reference to the guideline that recommends it.

How do I submit a reimbursement claim after a PET-CT scan?

Before leaving the imaging centre, collect the original itemised bill and payment receipt. Combine these with your oncologist's referral, all supporting reports submitted with it, a completed claim form from your insurer, and your policy document. Submit at your insurer's nearest branch or through their portal. Most policies set a time limit for submitting claims after the service date — check this before your scan appointment. If submitting physical documents, keep photocopies of everything before handing them over, as original documents are not always returned.
