Why PET-CT Insurance — Claims Get Rejected

Your scan may be done, but that does not guarantee the money comes back. Knowing why claims fail — and what documents stop a rejection before it happens — is what separates a paid claim from a dispute that drags for months.

Medically reviewed by Dr. C. Raghavendra Reddy, Medical Oncologist, MBBS (Gold Medal) · DNB · DM (Medical Oncology, Gold Medal) · Last reviewed September 2026

  • Pre-auth skipped — Most rejections trace back to one step missed before the scan date. Pre-authorisation is mandatory at nearly every insurer.
  • Referral wording too vague — Standard referral letters are routinely queried. The specific clinical reason for the scan must be stated plainly.
  • Records incomplete — One missing document — a biopsy report, a discharge summary — is enough for a TPA to hold the whole claim.
  • Most rejections can be reversed — A well-documented appeal overturns a high proportion of initial rejections. The documents needed are usually already in your file.

PET-CT claims are rejected most often because pre-authorisation was not obtained before the scan, the referral letter does not state the clinical indication clearly, or supporting records are missing. Most of these rejections can be overturned on appeal. The documents that make a claim stick are the same records your oncologist already holds.

At CION, a whole-body PET-CT starts from Rs 10,499 — among the lowest published prices in Hyderabad. Indicative price, as of September 2026.

What does a PET-CT scan cost, and what will your insurer pay?

Scan typeIndicative cost (2025)Typical insurer position
Whole-body FDG PET-CT₹12,000 – ₹22,000Covered under most comprehensive policies when pre-authorised and medically indicated
Limited-region FDG PET-CT₹9,000 – ₹15,000Covered if the referral states a specific clinical indication
Specialised PET-CT (e.g. PSMA)Higher — confirm before bookingCovered by fewer policies; check your policy wording and empanelled centre list before booking
Repeat or surveillance scanSimilar range to first scanHigher documentation bar; evidence that the scan changes clinical management is required

How do you make sure your PET-CT claim is approved before the scan?

  1. Ask your oncologist for a referral that names the indication

The letter must state specifically why PET-CT is needed — for example, 'staging of confirmed lung cancer prior to planned treatment'. 'For evaluation' alone will be queried.

  2. Confirm the imaging centre is on your insurer's empanelled list

A scan at an unapproved centre is almost always rejected even with correct documents. Check the empanelled list on your insurer's website or call the TPA before booking the date.

  3. Submit the pre-authorisation request with the full document set

Send the referral, your diagnosis documents, relevant previous imaging reports, and your policy details together. A partial submission delays approval and can miss your scan window.

  4. Keep the written pre-authorisation approval letter

This is the most important document at claim time. Without it, cashless claims are rejected at the desk and reimbursement claims are queried at every stage.

  5. Submit the claim within your policy's time window

Most policies require reimbursement claims within 30 to 90 days of the scan. Late submission is a separate rejection ground from missing documents — check your specific policy.

Which documents make a PET-CT claim succeed or fail?

DocumentWhat to includeWhy claims fail without it
Oncologist's referral letterSpecific clinical indication, on letterhead, with doctor's registration numberVague referrals are the single most common rejection ground
Pre-authorisation approvalWritten TPA approval obtained before the scan dateClaims without pre-auth are rejected at most insurers regardless of other documents
Diagnosis confirmationBiopsy or histopathology report confirming the diagnosisEstablishes that the scan is for a covered condition
Previous imaging reportsCT, MRI, or ultrasound from the same treatment periodShows what prior imaging found and why PET-CT is a necessary next step
Treatment recordsDischarge summaries and treatment notes, recentClaims without them appear to be isolated requests, not part of ongoing care
Scan report and imagesRadiologist's written report and CD of imagesRequired for reimbursement; some TPAs ask for images as well as the report

Why do most PET-CT insurance claims get rejected?

The most common reason is that pre-authorisation was not obtained before the scan. Once the scan is done without approval, most policies treat it as an unapproved procedure and exclude it from reimbursement regardless of medical need.

The second most common reason is a referral letter that does not state the clinical indication clearly. Insurers assess medical necessity from the documents you submit, not from what your doctor intended. 'For cancer evaluation' is not the same as 'staging confirmed non-small-cell lung cancer prior to planned treatment'.

Missing supporting records are the third major cause. One absent document — a pathology report, a previous imaging summary — is enough for a TPA to return the entire claim file.

What do you do when a PET-CT claim is rejected?

Read the rejection letter carefully. It will name the specific reason — missing document, no pre-auth, not medically necessary. That reason tells you exactly what to gather.

Write a formal reconsideration to the TPA's grievance team. Attach the document the rejection letter named, plus any clinical notes from your oncologist that address the stated objection directly.

If the reconsideration is also rejected, you can file a complaint with the Insurance Ombudsman in your state at no cost. IRDAI maintains a list of regional ombudsman offices. Most documentation disputes are resolved at this stage without going further.

Did you know?

Most PET-CT claim rejections are procedural — a missing document, a pre-authorisation gap, a vague referral. They are not clinical denials of coverage.

The Insurance Ombudsman is a free service maintained by IRDAI that resolves disputes insurers and policyholders cannot settle directly. Filing a complaint is a right, not a last resort.

Common questions

Frequently asked questions

Can I claim reimbursement for a PET-CT scan done without pre-authorisation?

Most policies treat a scan done without pre-authorisation as an unapproved procedure and exclude it from reimbursement, even if the scan was medically necessary. Some policies allow post-facto approval for genuine emergencies, but a planned oncology PET-CT does not usually qualify. The only reliable approach is written approval before the scan date, even if that means a short delay.

What does the oncologist's referral letter need to say?

The referral must name the patient, state the confirmed diagnosis with the tumour site, and give a specific clinical reason for the scan — for example, 'staging of confirmed lung cancer prior to planned surgery' or 'assessing treatment response after the third cycle'. The requesting doctor's registration number, signature, and hospital letterhead are all required. A referral dated several months earlier may be queried for currency.

How do I know whether my policy actually covers PET-CT?

Check the specific exclusions list in your policy document, not only the inclusions. Most comprehensive indemnity policies issued after 2019 cover PET-CT, in line with IRDAI guidelines on day-care procedures. Older policies and some benefit-based plans may not. If the wording is unclear, ask your TPA the question in writing before you book — a phone call does not give you a record you can use later.

How long does a reconsideration or appeal take?

IRDAI requires insurers to acknowledge a written complaint within 15 days and resolve it within 30 days. TPA practice varies. If you have not received a written response within the regulatory window, that missed deadline itself becomes grounds for an ombudsman complaint. Keep a record of every document you submitted and the date you sent it, so you can show the timeline if needed.

What is the Insurance Ombudsman and is it free to use?

The Insurance Ombudsman is an independent authority set up by IRDAI to settle disputes between policyholders and insurers, at no cost to the policyholder. Each state has at least one regional office; complaints can be filed by post, in person, or through the IRDAI Integrated Grievance Management System online. It can direct an insurer to honour a claim. It is a meaningful escalation step, not a formality.

Can CION help with the documents needed for pre-authorisation?

The clinical team at CION can provide the documentation your insurer needs — referral letters with specific clinical indications, treatment summaries, and investigation reports. PET-CT scans at CION are coordinated with partner imaging centres. The administrative steps of submitting to your TPA and following up on approval remain with you as the policyholder, though the team can respond to specific queries your insurer raises about the clinical reasoning.
