CION Cancer Clinics
Betel nut, pan masala and your liver: what is known | CION Cancer Clinics
Areca nut, the base of supari, pan masala and gutka, is a proven cause of mouth cancer. Research, mostly from East Asia, also links long-term chewing to liver scarring and liver cancer, especially alongside hepatitis B or C. This page explains what is in these products, how strong the liver evidence really is, and practical steps to stop. At CION Cancer Clinics, our specialists help you understand your own risk and plan regular liver checks that fit it.

The short answer
Can betel nut or pan masala damage the liver?
Possibly, and the evidence is still growing. Areca nut, the main ingredient in supari, pan masala and gutka, is a proven cause of mouth cancer. Studies, mostly from East Asia, have also linked long-term chewing to liver scarring and liver cancer, especially in people who already carry hepatitis B or C.
Which liver cancer this page is about
This page is about primary liver cancer, the kind that starts in the liver, mostly hepatocellular carcinoma (HCC, the cancer of the main liver cells). If a scan has shown cancer that spread to the liver from another organ, the questions are different. Read the two very different things called liver cancer first.
Why the liver is involved at all
When you chew areca nut, its chemicals are absorbed through the lining of the mouth and swallowed into the gut. Both routes lead to the blood, and the liver is the organ that processes what reaches the blood. Over many years, that steady load may add to inflammation in a liver that is already under strain.
A risk factor raises the chance across a whole group of people. It does not decide what happens to any one person.Know what you are chewing
What is actually in supari, pan masala and gutka?
These products look different but share one ingredient. What changes is what is added to it.
Areca nut (supari, vakka)
The nut of the areca palm, sold raw, roasted, sliced or sweetened. It is the common base of every product on this page and is classed as a cause of cancer in humans.
Betel quid (pan, killi)
Areca nut wrapped in a betel leaf with slaked lime, and often catechu, spices or tobacco. Chewing it without tobacco is still chewing areca nut.
Pan masala
A dry, packaged mix of areca nut, catechu, lime and flavourings. It is often sold as tobacco-free, which only means it has no tobacco. The areca nut is still there.
Tobacco-free is not the same as harmless.Gutka and mawa
Pan masala with tobacco added. These carry the harm of areca nut and the harm of chewed tobacco together. Gutka is banned for sale in many Indian states, but it is still widely used.

Ask an oncologistWhat the research shows
How strong is the link between areca nut and liver cancer?
The link with mouth cancer is firm and accepted worldwide. The link with the liver is weaker and newer. Most of the research comes from Taiwan, where betel quid chewing is common and so is hepatitis B. Those studies found more cirrhosis and more liver cancer among long-term chewers.
The hepatitis connection
The risk looked highest in people who chewed and also carried hepatitis B or C. That is the group most relevant to families in Telangana and Andhra Pradesh. If you carry hepatitis B, stopping supari is one more way to take pressure off your liver. Read what a hepatitis B carrier's cancer risk really is.
What we honestly do not know
There is very little Indian research on areca nut and the liver. Chewers often also smoke, drink or use tobacco, which makes it hard to separate one habit from another. The major cancer organisations do not yet list areca nut among the main liver cancer risk factors. We think it is wise to stop, but we will not pretend the numbers are known.
Who this does not apply to
An occasional sweet pan after a wedding meal is not what these studies looked at. The concern is daily chewing over many years.

What to do now
How do I stop chewing supari or pan masala?
Be honest with your doctor
Say what you chew and how many packets or pieces in a day. Your doctor asks to plan your care, not to judge you.
Look at your mouth
Check for white or red patches, sores that do not heal, or trouble opening your mouth wide. Any of these needs a dentist or doctor soon, whatever happens with the liver.
Get tested for hepatitis B and C
If you have chewed for years and never had a hepatitis test, ask for one. It is a simple blood test and it tells you which liver risks matter most for you.
Replace the habit, not just the product
Many people chew after meals, at work or with friends. Plain fennel seeds, a short walk or a glass of water at those moments helps more than willpower alone. The national tobacco quitline and hospital cessation clinics help with areca nut too.
Keep regular liver checks if you need them
Stopping helps, but it does not replace scans if you have cirrhosis or hepatitis B. See who needs liver cancer screening.
Commonly believed
What families often say about supari and pan
Chewing anything after a meal makes saliva flow, which is why it feels helpful. Areca nut itself is a cancer-causing substance. There is no evidence it protects the liver.
It is safer than gutka only in having no tobacco. The areca nut in it still causes mouth cancer and may strain the liver over many years.
Sweetened and flavoured supari is still areca nut. Children who start young often keep chewing for life. It is better not to offer it at all.
The leaf is not the main concern. The areca nut, the lime and any tobacco inside the pan are. Tradition and safety are separate questions.
Being straight with you
What this page cannot tell you
It cannot tell you your own risk of liver cancer. That depends far more on hepatitis B or C, alcohol, weight, diabetes and whether your liver is already scarred. Areca nut is one possible part of the picture, and the size of that part is not yet known.
It cannot read your scan or report
If you have been told about a spot on the liver, fatty liver or cirrhosis, you need a doctor who can see the actual report. CION arranges scans through diagnostic partners, and the tumour board looks at them with your full history.
It cannot assess your mouth
Long-term chewers should have their mouth checked regularly. That is a separate examination by a dentist or a head and neck specialist, and it matters more than anything on this page. See the oral cancer guide.
If you chew daily and also have hepatitis, cirrhosis or fatty liver, call the helpline. Someone will tell you which checks are worth arranging.Questions we are asked
Common questions about betel nut and the liver
Does betel nut cause liver cancer?
It may add to the risk, particularly in people who already carry hepatitis B or C. The evidence comes mostly from studies in East Asia and is not as strong as for mouth cancer. It is reasonable to treat daily chewing as a habit worth stopping for the liver as well as the mouth.
Is pan masala worse than plain supari?
Both contain areca nut, which is the main concern. Packaged pan masala adds flavourings and catechu, and is easy to chew all day. Gutka adds tobacco on top. None of them is a safe version, and the amount you chew matters more than the brand.
I chew daily and have hepatitis B. What should I do?
Tell the doctor who manages your hepatitis B and try to stop chewing. Keep taking any antiviral tablets you have been prescribed and keep your scans on schedule. Hepatitis B is controlled, not removed, so these checks continue even after you stop.
Can stopping now reverse the damage?
The liver can recover from a lot of strain if it is not yet badly scarred. Stopping removes one source of that strain. Scarring that has already become cirrhosis does not fully reverse, but stopping still helps protect the liver you have.
Does areca nut affect liver blood tests?
Chewing on its own is not known to cause a typical pattern on liver function tests. If your tests are abnormal, your doctor will look for the usual causes first, such as hepatitis, alcohol, fatty liver or medicines. Mention the chewing so they have the full picture.
Is betel leaf alone harmful?
The betel leaf by itself has not been shown to cause cancer in the way areca nut has. The concern is what goes inside the pan: areca nut, slaked lime and often tobacco. A pan without areca nut is rare in everyday use.
Should my family members who chew be checked?
A mouth check is the first priority for anyone who chews regularly. A hepatitis B and C blood test is also sensible for every adult who has never had one. Read should family members be screened for more on liver checks.
Where can I get help to stop?
Tobacco cessation clinics in many hospitals also help people stop areca nut and pan masala, and the national quitline can guide you by phone. Your CION doctor can talk it through at a consultation. Call the CION helpline if you are unsure where to begin.
