Skin care when you are itching constantly | CION Cancer Clinics

Cool water, a thick unscented moisturiser, loose cotton clothes, short nails and a cool bedroom can all ease the itching that comes with jaundice. They protect the skin while your doctors treat the cause, which is often a blocked bile duct. This page explains a simple daily routine, the home habits that make itching worse, and the signs that mean a doctor today rather than another cream. At CION Cancer Clinics, our team supports patients and families through symptoms, follow-up and life during and after treatment.

The short answer

Can anything really help skin that itches all day and all night?

Yes. Itching from jaundice is real, it is not in your head, and good skin care can take the edge off it while your doctors treat the cause. Cool water, a thick unscented moisturiser, loose cotton clothes, short nails and a cool bedroom all make a difference. None of them replaces the medical side of the plan, but together they protect the skin from the damage that scratching does.

Why the skin itches when bile is blocked

When the flow of bile is blocked or slowed, a problem doctors call cholestasis, substances that should leave through the bowel build up in the blood instead. They act on nerve endings in the skin. That is why the itch is often worst on the palms and soles, and why it gets worse at night and in heat. You can read more on itching from jaundice and what actually helps.

Why skin care still matters when the cause is inside

Scratching breaks the skin. Broken skin lets germs in and itches even more as it heals, which starts a cycle. Looking after the skin breaks that cycle, and it is the part a family can do at home tonight.

Many patients are told nothing can be done for this itch. That is not true. Ask again.

What helps the skin

Which skin care habits actually ease jaundice itching?

These are simple, cheap and safe for most people with liver disease. Try them together rather than one at a time.

Cool, not hot, water

Hot baths feel good for a moment and then make the itch worse. Bathe in cool or lukewarm water, keep it short, and pat the skin dry with a soft towel instead of rubbing.

Moisturise, often

Dry skin itches more. Apply a thick, plain, unscented moisturiser straight after bathing while the skin is still a little damp, and again whenever it feels tight.

Small things that help

  • A tube kept in the fridge feels soothing
  • Plain creams are safer than perfumed ones
  • Ask before using any medicated cream

Soft, loose cotton

Wool, synthetic fabric and tight elastic rub the skin. Loose cotton clothes and cotton bedsheets let the skin breathe. Wash them in a mild soap and rinse well.

A cool room at night

Heat and sweat make cholestatic itching worse. A fan, a light sheet and a room kept as cool as the season allows can mean the difference between some sleep and none.

Not sure whether this applies to you?

A day of skin care

What does a simple daily routine look like?

Morning wash

Use cool water and a mild, soap-free cleanser or a gentle bar. Skip strong soaps, antiseptic liquids and scrubs. Pat the skin dry.

Moisturiser straight away

Spread a generous layer over the arms, legs, back and the soles of the feet. A family member can help with the back, where the itch is often hardest to reach.

Nails kept short

Trim and file the nails so scratching does less harm. If the itch is bad at night, thin cotton gloves stop the skin being torn during sleep.

Something to do instead of scratching

Press or pat the itchy spot, or hold a cool damp cloth on it. Keeping the hands busy with something also helps more than people expect.

Evening cool-down

Moisturise again before bed, wear loose cotton, and keep the room cool. Note down how the night went, so you can tell your doctor whether the medicines are working.

!
When itching comes with something more serious

If the yellow colour comes on suddenly with fever and shivering, go to hospital today. If you have a bile duct stent and you get a fever, go to hospital today and say you have a stent. See a doctor this week if scratched skin turns red, hot, swollen or starts to leak pus. New confusion or drowsiness needs hospital today. None of these are skin problems, and no cream will fix them.

Commonly believed

What families often try, and what is actually true

"An allergy tablet will stop the itching."

This itch is not an allergy, so ordinary allergy tablets often do little apart from making the person sleepy. Sleepiness can be a problem in someone with a damaged liver, so ask before taking them. Doctors have other medicines that work on the real cause.

"A hot bath with strong soap will clean it away."

The itch comes from the blood, not from dirt on the skin. Hot water and strong soaps strip the skin's natural oils and make the itch worse.

"Home pastes and oils from a neighbour are harmless."

Some home remedies irritate broken skin, and some herbal products taken by mouth can harm a damaged liver. Plain unscented moisturiser is the safe choice. Tell your doctor about anything else you are using, without worrying about being judged.

"Nothing can be done, you just have to bear it."

Several things can help. Relieving a blocked bile duct often eases the itch, and there are medicines your doctor can try one after another. Skin care makes the wait easier.

Being straight with you

What this page cannot tell you

It cannot tell you why your skin is itching. Itching with jaundice can come from a blocked bile duct, from the liver itself not working well, or sometimes from a medicine. Each has a different fix. Your doctor will work out which it is with blood tests and a scan, and decide whether a stent or drainage to relieve the jaundice is the first step.

It cannot choose a medicine for you

Medicines for this itch need care in someone with liver disease. Your oncologist or hepatologist, meaning a liver specialist, will choose them and adjust them. Our page on medicines for cholestatic itching explains the options in general.

Who this does not apply to

If your itch comes with a rash, hives or blisters, or started with a new treatment, it may be a reaction rather than jaundice. Some cancer medicines cause itching and rash of their own. See dry, itchy skin during chemotherapy, and tell the treating team the same day.

Questions we are asked

Common questions about skin care and itching

Why is the itching worse at night?

The skin warms up under blankets, there is nothing to distract you, and cholestatic itching often feels stronger after dark. A cool room, cotton bedding, moisturiser before bed and short nails all help. Tell your doctor if you are not sleeping, because that alone is a reason to change the plan.

Which moisturiser should I buy?

A thick, plain, unscented cream or ointment from any chemist is usually enough. Avoid strong perfume, alcohol-based lotions and products that promise a medical effect. If the skin is broken or infected, ask your doctor or pharmacist before putting anything on it.

Can calamine or menthol creams help?

Some people find a mild cooling cream soothing for a short while. Calamine can dry the skin, so follow it with moisturiser. Stop anything that stings or makes the skin red. These creams only calm the surface and do not touch the cause inside.

Is it safe to use coconut oil?

Plain coconut oil on unbroken skin is generally harmless and many families use it. It does no more than an ordinary moisturiser. Avoid oils mixed with herbs, perfumes or camphor on broken skin, and never take any oil or herbal mixture by mouth as an itch remedy without asking your doctor.

Will the itching go away once the jaundice clears?

Often it eases as bile starts flowing again, for example after a blocked duct is opened with a stent. When the itch comes from the liver itself not working well, it may settle less. Your doctor can tell you which situation you are in and what to expect.

My father scratches until he bleeds. What should we do?

Keep his nails short, try cotton gloves at night, and keep the broken areas clean and loosely covered. Tell his doctor soon, because this level of itching needs a medicine review, not just more cream. If the scratches look infected or he has a fever, see a doctor without delay.

Does eating differently help the itch?

No particular food is known to stop it. What helps is eating well overall, including enough protein, because families often cut protein when the patient needs it. Drinking enough water also keeps the skin from drying, unless your doctor has asked you to limit fluid.

Who do I call if nothing is working?

Call your treating oncologist or the CION helpline and describe how bad the itch is and how you are sleeping. There are several medicines to try, and a blocked duct may need to be looked at again. You should not be left to cope with it alone.
