Can right shoulder blade pain mean liver cancer? | CION Cancer Clinics

Right shoulder blade pain is rarely caused by liver cancer. It is usually a strained muscle, a neck problem or gallstones. The liver can send pain to the right shoulder, but when it does, other signs usually come with it, such as an ache under the ribs, nausea or yellow eyes. This page explains the common causes, how to tell them apart, and when to see a doctor. At CION Cancer Clinics, our team goes through your scan, blood test and biopsy reports with you and plans the next step.

The short answer

Can pain in the right shoulder blade come from the liver?

It can, but in most people it does not. Pain behind or under the right shoulder blade is far more often a strained muscle, a stiff neck, a problem with the spine, or gallstones irritating the gallbladder. Liver cancer is one of the least likely explanations, and it rarely shows itself this way on its own.

Why the liver can make the shoulder hurt

The liver sits under the right ribs, just below the diaphragm, the sheet of muscle you breathe with. The nerve that supplies the diaphragm starts in the neck, next to the nerves for the shoulder. When something stretches the liver's outer covering or presses on the diaphragm, the brain can read it as shoulder pain. Doctors call this referred pain.

What usually comes with it when the liver is involved

Pain referred from the liver or gallbladder rarely arrives alone. It tends to come with an ache or fullness under the right ribs, pain after fatty meals, nausea, yellow eyes, dark urine, or weight loss nobody can explain. Shoulder pain that gets worse when you move your arm, and eases with rest, points to the muscles and joints instead.

Shoulder pain alone is not a sign of liver cancer. Shoulder pain with other liver symptoms deserves a doctor's visit.

Most likely first

What usually causes pain under the right shoulder blade?

Doctors work through the common explanations before the rare ones. Most of these are treatable and none of them is cancer.

Muscle strain and posture

Long hours on a phone or two-wheeler, lifting, or sleeping awkwardly can strain the muscles around the shoulder blade. The pain changes with movement and is tender when pressed.

Neck and spine problems

Wear in the neck bones or a pinched nerve can send pain down to the shoulder blade, sometimes with tingling in the arm.

Gallstones

The classic cause of right shoulder pain from the abdomen. It often comes after a heavy or oily meal, with pain under the right ribs and nausea. See gallstones and cancer fear.

Less common, still not cancer

  • A liver abscess or hepatitis, usually with fever
  • Fatty liver that has enlarged the liver
  • Chest problems such as pleurisy or pneumonia
  • Acidity or a stomach ulcer

And sometimes, a liver tumour

A large tumour, or cancer that has spread to the liver from elsewhere, can stretch the liver's covering. By then there are almost always other signs too.

Not sure whether this applies to you?

Ask an oncologist
!
When this cannot wait

Go to hospital today if shoulder or chest pain comes with breathlessness, sweating or a tight chest, because the heart can cause left or right shoulder pain. Go today too for severe pain under the right ribs with fever and shivering, or for yellow eyes with fever. See a doctor this week if the pain keeps coming back with nausea, weight loss, yellow eyes or a swollen abdomen.

At the clinic

What will the doctor do to find the cause?

  1. Ask about the pain

When it started, what makes it worse, whether meals or movement bring it on, and whether you have hepatitis B or C, diabetes, fatty liver or a history of drinking. Answer honestly. Nobody is judging.

  2. Examine the shoulder and the abdomen

Pressing on the muscles, moving the neck, and feeling under the right ribs for tenderness or an enlarged liver usually separates a muscle problem from an abdominal one.

  3. Blood tests if the abdomen is suspected

A liver function test and a blood count. Read more on reading a liver function test.

  4. An ultrasound of the abdomen

This is usually the first scan. It shows gallstones, fatty liver and most liver lumps. CION arranges scans through its diagnostic partners.

  5. A CT or MRI only if something needs a closer look

If the ultrasound finds a spot on the liver, a contrast scan tells doctors much more. The spot on the liver page explains what happens then.

Side by side

Is my shoulder pain from a muscle or from inside the abdomen?

Points to muscle or spine Points to the liver or gallbladder
Worse when you move the arm or neck Unchanged by movement, may follow meals
A tender spot you can press on No tender spot on the shoulder itself
Eases with rest, heat or a pain balm Comes with an ache under the right ribs
You feel otherwise well Nausea, fever, yellow eyes or weight loss

Commonly believed

What families often believe about right shoulder pain

"Right shoulder pain means something is wrong with the liver."

Most right shoulder pain comes from muscles, joints and the neck. The liver becomes a question only when other abdominal symptoms come along with it.

"If it were liver cancer, pain would be the first sign."

Early liver cancer usually causes no pain at all. That is why people with cirrhosis or hepatitis B are offered regular surveillance scans even when they feel well. See why early liver cancer is often silent.

"A liver tonic will settle the pain."

Tonics and detox powders do not treat gallstones, muscle strain or a liver tumour. Some herbal products can harm a liver that is already damaged. Tell your doctor about anything you take.

"A normal liver test means the liver is fine."

Liver blood tests can be normal with a small lump, and abnormal for reasons that are not serious. The doctor reads them together with the examination and the scan.

Being straight with you

What this page cannot tell you

It cannot tell you what is causing your pain. Only an examination, and sometimes a blood test and an ultrasound, can do that. Reading lists of causes at night tends to make the rare ones feel likely. They are not.

Who this page is not written for

If you already have a liver cancer diagnosis and new pain under the ribs or in the shoulder, tell your oncologist rather than reading general advice. New pain during treatment can mean the plan needs adjusting, and pain can be controlled well. If you have cancer elsewhere and a scan mentions the liver, the cancer in the liver page explains the difference between primary and secondary liver cancer.

When to ask for a specialist opinion

If an ultrasound shows a lump on the liver, or you live with cirrhosis or hepatitis B and the pain is new, an oncologist or hepatologist should see the reports. CION's team can review your scans and tell you plainly whether anything more is needed.

Bring every report and scan CD you have. It saves a repeat test.

Questions we are asked

Common questions about right shoulder pain and the liver

Is right shoulder blade pain a sign of liver cancer?

Rarely, and almost never on its own. Most right shoulder blade pain is from muscles, the neck or gallstones. A liver tumour large enough to cause referred pain usually brings other signs too, such as fullness under the ribs, weight loss or yellow eyes. If those are present, see a doctor this week.

What does liver pain feel like?

Usually a dull ache or heaviness under the right ribs, sometimes spreading to the back or right shoulder. It is rarely sharp. A sharp pain that catches when you breathe in is more often from the chest wall or lungs, and a cramping pain after meals is more often the gallbladder.

Can gallstones cause right shoulder pain?

Yes. This is one of the commonest abdominal causes. The pain often comes in episodes after oily or heavy meals, lasts a while, then settles. An ultrasound shows gallstones clearly. Severe pain with fever needs hospital the same day, because an infected gallbladder needs treatment quickly.

Can fatty liver cause shoulder pain?

Fatty liver usually causes no pain. Some people feel a heaviness under the right ribs when the liver is enlarged. Shoulder pain from fatty liver alone is unusual, so the doctor will look for another cause. Fatty liver still deserves attention, because over many years it can scar the liver.

Should I get a scan for shoulder pain?

Not usually as a first step. If the pain behaves like a muscle problem, rest and simple treatment come first. A doctor will suggest an abdominal ultrasound if the examination or your other symptoms point to the liver or gallbladder, or if you have liver disease already.

I have hepatitis B. Should new shoulder pain worry me?

It is still most likely muscular. Because hepatitis B raises liver cancer risk, mention it to the doctor who monitors your hepatitis. Keep up your regular surveillance ultrasound whether or not you have pain, since early liver cancer is usually silent.

Can cancer elsewhere cause right shoulder pain?

Occasionally. A lung tumour near the top of the lung, or cancer that has spread to the liver or bones, can cause it. These are uncommon, and they usually come with other symptoms such as a long cough, weight loss or night sweats. A doctor can check for these sensibly.

Who should I see first?

Your family doctor or a general physician. They can examine you and decide whether you need tests. If a scan shows a lump on the liver, call the CION helpline and our team will tell you which specialist should see the reports.

Your Specialists
