The short answer

Should I choose cashless or reimbursement for liver cancer treatment?

Choose cashless when the hospital doing your procedure is in your insurer's network, because the insurer pays the hospital directly and you pay only what is not covered. Choose reimbursement when it is not, or when the treatment is outside the hospital, and keep every paper so you can claim the money back afterwards.

Why liver cancer makes this harder than most

Liver cancer treatment is rarely one bill. A family may face a scan, a biopsy, TACE, ablation or surgery, then months of tablets or immunotherapy, then follow-up scans. Each step can happen at a different hospital, and each step can need a different claim route. Many families end up using both cashless and reimbursement in the same year.

This page is general information

It is not legal or financial advice. Your policy wording decides what is paid. Your insurer, its TPA (the third-party administrator that handles claims) and the hospital's insurance desk are the people who can confirm it for your own case.

Side by side

How are cashless and reimbursement claims different?

Cashless Reimbursement
Insurer pays the network hospital directly You pay the hospital, then claim the money back
Needs approval before admission Needs a full claim with original papers after discharge
Only at hospitals in your insurer's network Works at most registered hospitals, in or out of network
You pay deductions and non-covered items at discharge You wait for the money, and deductions come later
Less money needed up front You need savings or a loan from family to bridge the gap

Indicative cost

How much might I have to pay up front if I go the reimbursement route?

These are 2026 Hyderabad estimates, not quotes. With reimbursement, this is roughly what you would need to have ready before the insurer pays you back.

₹75,000 – ₹2,50,000

TACE, per session

Often repeated, so the up-front amount comes round more than once.

₹3,00,000 – ₹6,00,000

Liver resection, open

The single largest bill most families face outside transplant.

₹80,000 – ₹3,00,000 or more

Systemic therapy, per cycle

Repeats with every cycle, so the total depends on how long treatment continues.

Indicative only. Transplant, Y-90, scans and biopsy are quoted separately by the treating hospital. Your out-of-pocket cost depends on your policy, its sub-limits and any scheme you hold. Ask for a written, itemised estimate before treatment starts.

Leave a number, we will call you

One field. No form to fill in, and no charge for the call.

Step by step through treatment

Which claim route usually fits which part of liver cancer care?

The answer can change from one step to the next. These are the usual patterns, not rules.

Scans and biopsy

Outpatient scans are often not covered unless your policy has outpatient cover, or unless they lead to an admission. Keep the bills. Some insurers pay tests done shortly before and after a hospital stay as part of that claim.

TACE, ablation and surgery

These are hospital procedures and suit cashless best when the hospital is in your network. Start pre-authorisation as soon as the date is fixed.

Immunotherapy and drips

Given in day-care, so many policies treat each cycle as a day-care claim. Cashless is often possible, but each cycle may need its own approval.

Tablets taken at home

Targeted tablets such as sorafenib or lenvatinib are taken at home. Some policies cover oral cancer medicine, often only by reimbursement and sometimes with a cap. Read the clause carefully. The cost of systemic treatment page explains what you may face.

Never stop or change tablets because a claim is pending. Talk to your oncologist first.

Getting paid

What do I need to do for each kind of claim?

Cashless: before admission

Give the hospital's insurance desk your policy card and ID. They send the treatment plan, estimate and reports to your insurer or TPA for pre-authorisation.

Cashless: at discharge

The hospital sends the final bill for approval. You pay the deductions, co-payment and items the policy does not cover before you leave.

Reimbursement: tell the insurer early

Most policies ask you to inform them of a planned admission beforehand, and of an emergency soon after. Late notice is a common reason for a cut or a refusal.

Reimbursement: collect the originals

Discharge summary, itemised bill, payment receipts, pharmacy bills, reports and the doctor's prescriptions. Ask the hospital for these before you go home.

Reimbursement: file on time

Send the claim form and originals within the time your policy sets. Keep photocopies of everything, and note the claim number.

Schemes and help

What about Aarogyasri, Ayushman Bharat and other schemes?

Government schemes such as Aarogyasri, Ayushman Bharat (PM-JAY), CGHS, ECHS and ESI work like cashless cover at empanelled hospitals. The hospital claims a set package amount from the scheme, so there is usually no reimbursement step for the family. Which liver cancer treatments are in the package list changes, so check at the time with the scheme desk. See does Aarogyasri cover liver cancer and Ayushman Bharat and liver cancer.

Who this does not apply to

If you have no insurance and no scheme, neither route applies. Ask about government schemes you may qualify for and the relief funds some families apply to. If transplant is being discussed, the claim picture is different again and is covered on its own page.

How CION helps

CION coordinates scans, procedures and surgery through accredited partner hospitals and gives immunotherapy, targeted therapy and chemotherapy at its own centres. Our team helps you gather reports, plans and forms for your insurer or scheme.

Being straight with you

What this page cannot tell you

It cannot tell you whether a particular hospital is in your network today, or what your policy will pay for a particular step. Networks change, and policy wording differs a great deal between insurers and even between plans from the same insurer. Check both with your insurer or TPA before each admission.

When a claim goes wrong

If a cashless request is refused, you can usually still pay and file for reimbursement. If a claim is cut, ask for the reason in writing, then use the insurer's grievance cell and, if you still disagree, the insurance ombudsman. A lawyer or a consumer forum can advise on anything beyond that. This page cannot.

Keep one folder for every bill, report and approval from the first scan onwards. See paperwork to keep from day one.

Questions we are asked

Common questions about cashless and reimbursement claims

Is cashless always better than reimbursement?

Not always. Cashless means less money up front, but it only works at network hospitals. If the hospital best placed to do your procedure is outside the network, reimbursement lets you go there and still claim. Choose the hospital for the treatment first, then the claim route.

My cashless request was refused. What now?

Ask the hospital's insurance desk why. Often it is missing papers, which can be sent again. If it is still refused, you can pay and file for reimbursement. A cashless refusal is not the same as a final claim refusal, and the insurer must look at a reimbursement claim on its own merits.

Can I use cashless for one TACE and reimbursement for the next?

Yes. Each admission is a separate claim, so the route can differ each time. Many families do this when they move between hospitals or cities. Keep every approval, bill and discharge summary together, because the insurer tracks how much of your sum insured is left.

Are tablets like sorafenib or lenvatinib covered?

Some policies cover oral cancer medicines, often only by reimbursement and sometimes with a cap. Others do not cover medicines taken at home at all. Read the policy wording for oral chemotherapy or outpatient cancer drugs, and ask your insurer in writing before you rely on it.

Why did I pay something at discharge with cashless?

Cashless covers what the policy allows. Items such as some consumables, room rent above your limit, co-payment and non-medical charges are usually paid by you. A room above your limit can reduce the whole bill in proportion, so ask about room limits before admission.

Do I need a doctor's letter for a reimbursement claim?

Usually the discharge summary and prescriptions are enough, but insurers often ask for more for cancer claims. A short note from your oncologist explaining the diagnosis and why the treatment was chosen can prevent queries. Ask for it before you leave the hospital.

Does an alcohol history affect my claim?

Some policies have an exclusion linked to alcohol, and how it is applied to liver cancer is often disputed. Be honest with your doctor and insurer. The page on the alcohol exclusion problem explains how this works and where to ask for help.

Can CION help me with the claim?

Yes. We can help you gather the reports, treatment plan and forms your insurer or scheme asks for, and help you get a written estimate from the partner hospital. The final decision rests with your insurer. Call the helpline and someone will guide you through it.

What moves the figure

What affects the cost

Four things change the total more than anything else.

The technique used

A shaped or image-guided delivery costs more than a conventional one, and is chosen on clinical grounds rather than preference.

How many sessions

The total is driven by the number of sittings or cycles, not by a single per-visit figure.

Supporting tests

Scans, blood work and pathology done alongside treatment are billed separately.

Your cover

Aarogyasri, CGHS, ECHS, EHS or cashless insurance usually change the out-of-pocket figure substantially.

Paying for it

Insurance, schemes and payment

What you actually pay usually differs a great deal from the sticker figure.
