CION Cancer Clinics
First signs of fluid building up in your abdomen | CION Cancer Clinics
The first signs of ascites are often a tighter waistband, weight that rises while your arms get thinner, and feeling full after a few mouthfuls. In cirrhosis or liver cancer this fluid usually comes from raised pressure in the liver's blood supply. This page explains what to notice, what the doctor will check, and which signs mean you should go to hospital today. At CION Cancer Clinics, our team supports patients and families through symptoms, follow-up and life during and after treatment.

The short answer
What are the first signs that fluid is building up in my abdomen?
The earliest signs are usually small. Your trousers or saree petticoat feel tight at the waist, your weight creeps up while your arms and face look thinner, and your belly feels full or heavy after a small meal. Many people notice their ankles swelling at the same time.
This fluid is called ascites. In someone with cirrhosis or liver cancer it usually comes from raised pressure in the portal vein, the large vein that carries blood from the gut into the liver. That pressure, together with kidneys that start holding on to salt, lets fluid seep into the space around the bowel. The page on why fluid collects explains this in more depth.
Most early swelling has an ordinary cause
Gas, constipation, a heavy meal and simple weight gain are far more common reasons for a fuller belly. Even so, if you have cirrhosis, hepatitis B or C, or a liver tumour, a change in your waist is worth telling your doctor about within the week. Fluid found early is usually easier to control with diet and tablets.
This page is about fluid from liver disease. Fluid from cancer spread on the lining of the abdomen behaves differently; see the two kinds of ascites. What to notice
Which early changes point to fluid rather than fat or gas?
No single sign proves fluid is there. A pattern of several together, building over days to weeks, is what doctors look for.
Weight up, muscles down
Fluid is heavy. The scale rises steadily even though you are eating less, while your arms, shoulders and face look thinner. This mismatch is one of the clearest early clues.
Clothes and belt tighter
The waistband that fitted last month now pinches. Families often spot this first, before the person feels unwell.
Often comes with
- Belly fuller at the sides when lying flat
- A belly button that looks flatter or pushed out
Full after a few mouthfuls
Fluid presses on the stomach. You feel full quickly, may get heartburn, and eat less. Over time this adds to muscle loss, so tell your team.
Swollen ankles and less urine
Ankle and leg swelling often appears alongside ascites. Passing less urine than usual, or darker urine, can mean the body is holding on to salt and water.
Breathlessness when lying flat can mean the fluid is pressing upwards, or that fluid has collected around a lung. Not sure whether this applies to you?
Ask an oncologistIf you have fluid in your abdomen and develop a fever, new belly pain or tenderness, new confusion or drowsiness, vomiting of blood, black stools, or you pass very little urine, go to hospital today. Fluid in the abdomen can become infected without much warning, and that infection is treated with urgent antibiotics. Do not wait for your next appointment and do not treat it at home.
After you tell your doctor
What will the doctor do to check for fluid?
Examine your abdomen
The doctor taps on the sides of your belly and listens for a dull sound, then asks you to roll over. Small amounts of fluid are easy to miss this way, so a scan usually follows.
An ultrasound scan
Ultrasound shows even small amounts of fluid and also looks at the liver and the portal vein. CION arranges this through its diagnostic partners.
Blood and urine tests
Kidney function, sodium, potassium, albumin and liver tests show how the liver and kidneys are coping and guide what treatment is safe.
Sometimes a small fluid sample
For new ascites, a thin needle may draw off a little fluid to check for infection and cancer cells. This is done through an accredited partner hospital. Treatment then usually starts with cutting salt and, if needed, water tablets.
On your report
What do the words on my scan or notes mean?
- Ascites
- Fluid collected in the space around the bowel inside the abdomen.
- Mild, moderate or gross ascites
- Mild is seen only on a scan. Moderate makes the belly visibly fuller. Gross means the belly is large and tense.
- Portal hypertension
- Raised pressure in the portal vein, usually because a scarred liver resists blood flow. It is the main driver of liver-related fluid.
- Pedal swelling
- Swelling of the feet and ankles, which often comes with ascites and has the same cause.
- Serum albumin
- A protein made by the liver that helps hold fluid inside blood vessels. A low level makes fluid more likely to leak out.

Commonly believed
What do families often get wrong about a swelling belly?
In liver disease a growing belly with thinner arms often means fluid, while muscle is being lost. It is a reason to tell the doctor, and it says nothing good about appetite.
The fluid follows salt. Cutting salt is what helps. Most people do not need to restrict water unless a blood test shows very low sodium; see whether to restrict water.
In people with cirrhosis, most ascites comes from portal pressure and the liver's weaker function. Only tests on the fluid and scans can tell your doctor which cause applies.
Some remedies can harm a cirrhotic liver or the kidneys. Tell your team about anything you take, so they can check it is safe alongside your treatment.
Being straight with you
What this page cannot tell you
It cannot tell you whether the swelling you see is fluid, or what is causing it. Only an examination, a scan and sometimes a fluid test can do that. It also cannot tell you what a new episode of fluid means for your cancer or your liver; your oncologist and hepatologist will explain that with your reports in front of them.
Who this page does not apply to
If you have no liver disease and your belly has been slowly getting larger for years, ordinary weight gain is the likely reason. If your ascites comes from ovarian, pancreatic or bowel cancer spreading to the abdominal lining, the treatment is different and is covered on the pages for those cancers.
Who to ask
Bring a note of your weight over the last few weeks and any changes you noticed to your next visit. CION's medical oncologists review new ascites as part of your care and coordinate scans, fluid tests and drainage through accredited partner hospitals.
If you are not sure whether a change can wait, call the helpline and describe it.Questions we are asked
Common questions about early ascites
How quickly does ascites build up?
It varies. In some people it creeps up over several weeks; in others it appears within days, often after an infection, a bleed or a change in the tumour. A sudden rise is a reason to call your doctor promptly, because it can signal something new that needs checking.
Can I feel the fluid myself?
Small amounts usually cannot be felt. As it grows, the belly feels firm, tight or heavy, and may bulge at the sides when you lie down. Pressing on it should not be painful. New pain or tenderness with fluid needs a doctor the same day.
Should I weigh myself every day?
Many teams ask you to, at the same time each morning, after passing urine and before breakfast. A steady daily rise is often the first measurable sign of fluid. Write the numbers down and bring them along. The page on weighing yourself daily explains what to look for.
Is swelling of the legs the same thing?
It has a similar cause, salt and water held in the body, and the two often appear together. Leg swelling alone can also come from other problems, such as the heart, the kidneys or a clot. Your doctor will check which applies, especially if only one leg is swollen.
Does the fluid need to be drained straight away?
Usually not. Small or moderate amounts are normally managed first with less salt and, if needed, water tablets. Draining is used when the belly is large and tense or when breathing is uncomfortable. CION coordinates drainage through accredited partner hospitals when it is needed.
Can treatment for liver cancer bring on fluid?
Sometimes. Some treatments can strain an already weak liver for a while, and fluid can appear or increase afterwards. Your team watches for this. Tell them about any new swelling after a procedure or a new medicine rather than waiting for the next scan.
Why is my doctor checking my kidneys?
The kidneys decide how much salt and water the body keeps, and they come under strain in advanced liver disease. Water tablets also affect kidney tests. Regular blood tests let the team adjust treatment safely before a problem builds up.
Who should I contact if I notice these signs?
Tell the doctor who looks after your liver or your cancer within the week. If you also have fever, belly pain, confusion, black stools or vomiting of blood, go to hospital today. Call the CION helpline if you are not sure who to approach.
Meet CION's liver cancer specialists. Bring your scan reports and liver blood tests.
Our medical and surgical oncologists plan liver cancer care together, weighing the tumour and how well your liver works before choosing surgery, a liver-directed procedure or medicines.
