Yellow eyes and skin: when jaundice means cancer | CION Cancer Clinics
Yellow eyes are usually caused by viral hepatitis, gallstones, alcohol, a medicine or a harmless inherited condition, not cancer. Cancer becomes more likely when jaundice comes on slowly, without pain, with itching, dark urine and pale stools. This page explains the common causes, the signs that need a doctor this week or today, and the tests that find where the problem is. At CION Cancer Clinics, our team goes through your scan, blood test and biopsy reports with you and plans the next step.
The short answer
Do yellow eyes mean liver cancer?
Usually not. Most yellow eyes in India are caused by viral hepatitis, gallstones, alcohol, a medicine, or a harmless inherited quirk in how the body handles bilirubin. Cancer is one cause on a long list, and it becomes more likely when the jaundice arrives slowly, without pain, and with itching, dark urine and pale stools.
What jaundice actually is
Bilirubin is a yellow waste pigment made when old red blood cells are broken down. The liver clears it into the bile, which drains through the bile ducts into the gut. Jaundice means bilirubin is building up in the blood. That happens when too much is made, when the liver cannot process it, or when the bile ducts are blocked.
Where cancer fits in
A tumour can cause jaundice by blocking a bile duct or by replacing so much healthy liver that it stops coping. The blockage can come from a bile duct cancer, a cancer at the head of the pancreas, a gallbladder cancer, or a liver tumour. Each is treated differently, which is why the first job is to find where the blockage is.
Jaundice is a sign that needs a cause found, not a diagnosis in itself.Far more likely
What else turns the eyes yellow?
Doctors work through these first, because they are what most people with jaundice actually have.
Viral hepatitis
Hepatitis A and E, spread through contaminated water and food, are the commonest cause in India. Fever, nausea and tiredness come first, then the yellow eyes. Most people recover fully with rest.
Hepatitis E is more dangerous in pregnancy and needs a doctor quickly.Gallstones
A stone that slips into the bile duct blocks it. This usually causes sharp pain under the right ribs, often with fever, and can be removed by an endoscopy.
Medicines, alcohol and herbs
Anti-TB tablets, some antibiotics, heavy paracetamol use, alcohol and some herbal tonics can all inflame the liver. Stopping the cause, under a doctor's guidance, often lets it settle.
Blood and inherited causes
Gilbert's syndrome makes the eyes slightly yellow during fasting or illness and is harmless. Conditions that break down red cells too fast, such as haemolysis, also raise bilirubin.
Not sure whether this applies to you?
Ask an oncologistJaundice comes with a high fever or shivering, severe pain in the upper belly, confusion or unusual drowsiness, vomiting blood or black stools, or if you have a bile duct stent and the yellowing returns with fever. An infected, blocked bile duct and a failing liver are both emergencies. Do not wait for a routine appointment. See a doctor this week for any new jaundice, even without pain, especially with itching, dark urine or pale stools.
Finding the cause
What tests will the doctor do for jaundice?
-
Questions and examination
When it started, whether there was pain or fever, what you drink, and every tablet, tonic and supplement you take. The doctor checks for a large liver, fluid in the belly and scratch marks from itching.
-
Blood tests
A liver function test shows how high the bilirubin is and which pattern it follows. Reading a liver function test explains each line. Hepatitis tests and a blood count usually go with it.
-
An ultrasound
This shows whether the bile ducts are widened, which points to a blockage, and looks for stones or a lump in the liver or pancreas.
-
A CT or MRI scan if a blockage is seen
A detailed scan maps where the duct is blocked and why. CION arranges these scans through diagnostic partners, and the tumour board reviews them when cancer is a possibility.
-
Relieving the blockage, then a diagnosis
If a duct is blocked, a stent is often placed first so the jaundice can settle. A tissue sample may be taken at the same time. The page on stenting or drainage explains this step.

Commonly believed
Four things families tell us about jaundice
That is true for most viral hepatitis, but not for jaundice caused by a blocked duct. Every new jaundice in an adult deserves at least a blood test and an ultrasound, so the cause is known.
No remedy unblocks a bile duct, and some herbal preparations add strain to an already struggling liver. Delay is the real harm. Tell your doctor what you have taken, and nobody will judge you.
Painless jaundice is exactly the pattern that needs a scan soon, because slow blockage by a growth often causes no pain at first. Pain more often points to a stone.
Cutting food weakens people at the time they most need strength. Most people with jaundice should keep eating normally, in small meals. Ask a doctor or dietitian before restricting anything.
Being straight with you
What this page cannot tell you
It cannot tell you why your eyes are yellow. The cause is found from your blood results and a scan, read by a doctor who has examined you. Two people with the same bilirubin level can have completely different problems, so comparing numbers with a relative or online tells you very little.
If cancer has already been found
Jaundice in someone already diagnosed with liver or bile duct cancer is a different question. It may come from a blocked duct, which can often be relieved, or from the liver itself struggling. The page on blockage jaundice versus liver-failure jaundice explains why that difference shapes the plan.
Who this does not apply to
Jaundice in a newborn baby is common and handled by the paediatrician. Yellowing during chemotherapy or immunotherapy should go straight to your treating team. And a young adult with fever, nausea and yellow eyes after drinking outside water most likely has hepatitis A or E and needs a physician, not a cancer specialist.
If a scan has shown a blockage or a lump, call the helpline and ask for the reports to be reviewed.Questions we are asked
Common questions about yellow eyes and cancer
Is jaundice an early sign of liver cancer?
Not usually. Early liver cancer rarely causes jaundice, because the remaining liver copes well. Jaundice tends to appear when a tumour blocks a bile duct or when much of the liver is affected. Bile duct and pancreatic cancers cause jaundice earlier, because they sit right on the drainage pipe.
What is painless jaundice?
Yellowing that comes on slowly with no belly pain, often with itching, dark urine and pale stools. It suggests a slow blockage of the bile duct, which can be a stone, a narrowing or a growth. It needs an ultrasound and a doctor within the week, not months of waiting.
Why is my urine dark and my stool pale?
When bile cannot reach the gut, the stool loses its brown colour and the bilirubin spills into the urine instead, turning it dark. Together they point to a blockage rather than a liver infection. The page on dark urine and pale stools explains more.
Can a blood test show whether jaundice is from cancer?
No single blood test can. The liver function test shows the pattern, and markers such as AFP or CA 19-9 can raise or lower suspicion, but they are also raised by other conditions. A scan, and often a tissue sample, is what answers the question.
I have hepatitis B and my eyes look yellow. What should I do?
See your physician or hepatologist this week, or today if you are drowsy, confused or have fever. A hepatitis B flare is the most common reason. It is controlled with tablets, and the doctor will also check that your regular liver ultrasound is up to date.
Can jaundice from a blocked duct go away?
Often, yes. A stent or a drain placed by a specialist lets the bile flow again, and the yellowing fades gradually. Relieving it usually comes before any cancer treatment, because several treatments need the bilirubin to be lower first.
Is the itching related to the yellow eyes?
Usually, yes. When bile backs up, substances that cause itching build up in the skin. It is often worse at night and on the palms and soles. Relieving the blockage is what helps most, and the page on itching all over the body covers it.
Who should I see first?
A physician or gastroenterologist is the right first step for new jaundice. If a scan has shown a blockage, a lump in the liver or pancreas, or a narrowed bile duct, call the CION helpline and a medical oncologist can review the reports and plan the next tests with you.
Meet CION's liver cancer specialists. Bring your scan reports and liver blood tests.
Our medical and surgical oncologists plan liver cancer care together, weighing the tumour and how well your liver works before choosing surgery, a liver-directed procedure or medicines.
