Breast enlargement in men: is it the liver? | CION Cancer Clinics
Breast growth in a man, called gynaecomastia, is usually not cancer. Weight gain, age, puberty and medicines cause most of it. Long-standing liver disease such as cirrhosis can also cause it, because a scarred liver cannot clear hormones properly, and some water tablets used for liver fluid do the same. This page explains the causes, the warning signs and what a doctor will check. At CION Cancer Clinics, our team goes through your scan, blood test and biopsy reports with you and plans the next step.

The short answer
Can liver disease make a man's breasts grow?
Yes, it can, and it is usually not cancer. Breast tissue growing in a man is called gynaecomastia. Most of the time it comes from ordinary things like weight gain, getting older, puberty or a medicine, and the liver has nothing to do with it.
Where the liver comes in
A healthy liver breaks down hormones and clears them from the blood. When the liver is badly scarred, a condition called cirrhosis, it does this job less well. The balance between male and female hormones shifts, and soft breast tissue can grow under one or both nipples. Some water tablets given for fluid in the abdomen can do the same thing.
What this page is about
This page is for men who have noticed breast swelling and are worried about their liver, and for families who have read that it can be a sign of liver cancer. A liver tumour causing breast growth on its own is rare. When gynaecomastia is linked to the liver at all, the usual reason is long-standing liver disease, which is worth finding and treating in its own right.
Breast swelling is a reason to see a doctor, not a reason to panic. It does not tell you whether you have cancer. Most of these are not cancer
What usually causes breast enlargement in men?
Doctors work through the common causes first. Many men have more than one at the same time.
Extra body fat
Fat on the chest can look like breast growth. Doctors call this pseudogynaecomastia. It feels soft and evenly spread, with no firm disc under the nipple. Losing weight usually helps.
Age and puberty
Hormone levels change in teenage boys and again in older men. A tender, rubbery disc under one or both nipples is common at both stages. In boys it often settles by itself.
Medicines
This is one of the most common causes. Several medicines can trigger it, including some heart and blood pressure tablets and some water tablets.
Often involved
- Spironolactone, often used for fluid in cirrhosis
- Some acidity and ulcer tablets
- Hormone treatment for prostate cancer
- Body-building steroids and some herbal products
Long-standing liver disease
Cirrhosis from hepatitis B or C, fatty liver or alcohol can upset the hormone balance. It often comes with other signs, such as spider-like veins on the chest or red palms.
Other hormone and gland problems
An overactive thyroid, kidney disease, and problems with the testes can all shift hormones. A blood test usually points to these.
Rarely, a tumour
A small number of tumours make hormones, including rare tumours of the testis, the adrenal gland and, very rarely, the liver. Male breast cancer is a separate thing and feels different.
Not sure whether this applies to you?
Ask an oncologistSee a doctor this week if the swelling is on one side only, feels hard or fixed, is growing quickly, or comes with nipple discharge or blood, a pulled-in nipple or skin dimpling. See a doctor this week too if it comes with yellow eyes, a swelling abdomen or swollen legs. Go to hospital today if you vomit blood, pass black stools, or become confused or very drowsy. Those are liver emergencies, and they cannot wait for an appointment.
At the appointment
What will the doctor check if my breasts have grown?
-
A conversation about your medicines and habits
Bring every tablet, powder, tonic and supplement you take, including herbal ones. Say honestly how much alcohol you drink. Nobody is judging you. The doctor needs it to find the cause.
-
An examination of the chest and the body
The doctor feels the breast tissue on both sides, checks the armpits, and looks for signs of liver disease such as yellow eyes, spider veins, red palms or fluid in the abdomen. The testes are usually examined too.
-
Blood tests
Liver function tests show how well the liver is working. Hormone, thyroid and kidney tests look for the other common causes. If there is known liver disease, an AFP test, a blood marker sometimes raised in liver cancer, may be added.
-
A scan if needed
A breast ultrasound or mammogram is arranged if a lump feels suspicious. An abdominal ultrasound looks at the liver. CION arranges these scans through accredited diagnostic partners.
-
A plan for the cause
Often the answer is changing a medicine or treating the liver disease. If cirrhosis is found, you will also be advised on regular liver scans, because a scarred liver needs watching.

Gynaecomastia or a breast lump that needs checking?
Commonly believed
Four things men worry about, and what is actually true
Almost always it does not. Weight, age and medicines are far more common causes. When the liver is involved, long-standing liver disease is the usual reason, and a tumour is rare.
Cirrhosis from hepatitis B, hepatitis C or fatty liver can cause it in men who have never touched alcohol. And many causes have nothing to do with the liver at all.
Do not stop any medicine on your own. Water tablets control fluid in the abdomen, and stopping suddenly can let it build up fast. Ask your doctor, who can often switch you to another tablet.
Men can get breast cancer, though it is uncommon. A hard, one-sided lump should be examined, whatever is happening with the liver.
Being straight with you
What this page cannot tell you
It cannot tell you why your breasts have grown, or whether your liver is healthy. Only an examination, blood tests and sometimes a scan can answer that. Two men with the same swelling can have completely different causes.
Who this does not apply to
If you are a teenage boy with mild, tender swelling and no other symptoms, this is very likely the normal hormone change of growing up. Your family doctor can reassure you. If you already know you have cirrhosis and take spironolactone, your liver team will usually already know the likely cause, so tell them and ask about alternatives.
Who to ask
Start with your family doctor or physician. If liver disease is found, a hepatologist manages the liver, and CION's oncologists see people whose liver scan or AFP test raises a question about cancer. If you have cirrhosis, read about what cirrhosis is and the other skin signs of liver disease, and check the list of medicines to avoid with cirrhosis before taking anything new.
If you are unsure whether your signs need a liver check, call the helpline and describe them.Questions we are asked
Common questions about breast enlargement and the liver
Is breast enlargement in men a sign of liver cancer?
Rarely. It is far more often caused by weight, age, a medicine or long-standing liver disease such as cirrhosis. Only a small number of liver tumours make hormones. A doctor can check your liver with blood tests and an ultrasound if there is any reason to suspect a problem there.
Why does cirrhosis cause breast growth?
A healthy liver breaks down hormones. A scarred liver does this less well, so female-type hormones become relatively stronger than male ones. The breast tissue responds by growing. The same hormone shift can cause spider veins, red palms, shrinking testes and loss of body hair.
Can spironolactone cause gynaecomastia?
Yes. It is a common side effect of this water tablet, which is often given for fluid in the abdomen. Do not stop it on your own, because fluid can return quickly. Ask your liver doctor whether a different tablet would suit you.
Does it go away?
Often it shrinks if the cause is removed, such as a medicine being changed or weight being lost. Swelling that has been there a long time may become firmer and not go back fully. A doctor can talk you through options if it bothers you.
It is painful. Is that a bad sign?
Tenderness is common with gynaecomastia, especially when it is new and growing. Pain on its own does not suggest cancer. What matters more is a hard lump on one side, nipple discharge or bleeding, or skin changes. Those need a doctor this week.
Which tests will I need?
Usually liver function tests, hormone and thyroid tests, and kidney tests. A breast ultrasound or mammogram is added if a lump feels suspicious, and an abdominal ultrasound if liver disease is likely. Your doctor will choose based on what they find on examination.
Can herbal tonics or gym supplements cause this?
Some can. Body-building steroids are a known cause, and some herbal products contain hormone-like substances or harm the liver. Bring every product you take to your appointment, so the doctor can look at all of them together.
Who should I see first?
Your family doctor or a physician is the right first step. If they find liver disease, a hepatologist takes over the liver care. If a scan or blood test raises a question about cancer, call the CION helpline and we will help you get an oncology opinion.
Meet CION's liver cancer specialists. Bring your scan reports and liver blood tests.
Our medical and surgical oncologists plan liver cancer care together, weighing the tumour and how well your liver works before choosing surgery, a liver-directed procedure or medicines.
