The short answer

What does fatty liver mean, in words my family can follow?

Fatty liver means extra fat has built up inside the liver. In Telugu it is often called కొవ్వు కాలేయం (kovvu kaalēyam), or simply "fatty liver", the English words most families already use. It is very common on routine ultrasound reports, and on its own it is not cancer.

Why this page is written this way

Many parents in Telangana and Andhra Pradesh read Telugu more easily than English, while the son or daughter reads the report. This page is in English, with the key Telugu words beside their meaning, so you can explain the report at home. A full Telugu edition of these pages is being prepared separately.

Where cancer fits in

Most people with fatty liver never develop liver cancer. The risk rises only when fat leads to inflammation and then to scarring over many years. This page is about the liver's own health. If a scan has found a spot on the liver, read our page on fatty liver and cancer risk and keep any follow-up scan you were asked to have.

Fatty liver is a warning sign about the liver and about sugar and weight. It is not a diagnosis of cancer.

Telugu and English

Which Telugu words will we hear for fatty liver?

కాలేయం (kaalēyam)
The liver. It cleans the blood, stores energy and helps digest food.
కొవ్వు కాలేయం (kovvu kaalēyam)
Fatty liver. On reports you may also see "steatosis" or "MASLD", the newer medical name for fatty liver linked to weight and sugar.
వాపు (vaapu)
Inflammation. When fat causes inflammation, doctors call it steatohepatitis. This is the stage that can lead to scarring.
కాలేయం గట్టిపడటం (kaalēyam gattipadatam)
Hardening of the liver. This is how families often describe fibrosis (scarring) and cirrhosis (advanced scarring).
షుగర్ / మధుమేహం (madhumēham)
Diabetes. One of the strongest reasons a fatty liver gets worse.
మద్యం (madyam)
Alcohol. It adds a second injury to a fatty liver.
కామెర్లు (kaamerlu)
Jaundice, yellow eyes or skin. Not a sign of simple fatty liver, so see a doctor promptly if it appears.

Why it happens

Why do so many people in our families have fatty liver?

Fatty liver usually comes from several everyday things acting together. None of them is anyone's fault.

Sugar and diabetes

High blood sugar and insulin resistance push the liver to store fat. Fatty liver is very common in people with diabetes or prediabetes.

Weight around the waist

Fat stored around the stomach is closely linked to fat in the liver. Many Indians develop fatty liver at a lower body weight than people elsewhere, so a slim person can still have it.

Often seen with

  • High blood pressure
  • High cholesterol or triglycerides
  • Little daily activity

Alcohol

Drinking adds fat and inflammation of its own. Combined with metabolic fatty liver, it speeds up scarring.

Other liver conditions

Hepatitis B or C, some medicines and some herbal products can also stress the liver. Your doctor may test for these to be sure fat is the only problem.

After the report

The scan says fatty liver. What should we do next?

Do not panic, and do not ignore it

Fatty liver is common and usually manageable. It is still a reason to see a physician, because it often travels with diabetes and blood pressure problems that need attention too.

Check sugar, cholesterol and hepatitis

Your doctor will usually ask for blood sugar, cholesterol and liver blood tests, and may test for hepatitis B and C.

Ask whether there is scarring

An ultrasound shows fat but not scarring. A stiffness scan, described on our FibroScan page, or a blood-based score can. CION arranges these through diagnostic partners.

Start small, steady changes

Gradual weight loss, a daily walk, fewer sweets and sugary drinks, and less or no alcohol all reduce liver fat. Our page on reversing fatty liver explains what helps most.

Ask whether monitoring is needed

Only people with advanced scarring or cirrhosis usually need regular cancer checks. Our page on who needs monitoring explains how doctors decide.

Commonly believed

What do Telugu families often believe about fatty liver?

"He is thin, so his fatty liver report must be wrong."

Many Indians store fat in the liver and around the waist even at a normal weight. A slim person can have a genuinely fatty liver, and it deserves the same attention.

"We must stop rice completely."

Rice itself is not the cause. What matters is the total amount eaten, how much sugar and fried food comes with it, and how active the person is. Smaller portions with more vegetables and dal usually help more than banning one food.

"Liver patients should avoid protein like eggs and dal."

For fatty liver, protein is part of a healthy diet and helps keep muscle while weight comes down. Do not cut protein unless a doctor tells you to.

"A liver tonic or detox will clean out the fat."

No tonic or detox has been shown to remove liver fat, and some herbal products can harm the liver. Tell your doctor about anything you take, without worrying about being judged.

Being straight with you

What can this page not tell you?

It cannot tell you how much fat or scarring your own liver has. The grade on an ultrasound report is a rough guide to fat only. A physician or hepatologist needs your blood tests, your other conditions and often a stiffness measurement to judge what it means for you. Our page on fatty liver grades explains what the grades show.

It cannot read a lesion on your report

If the report mentions a lesion, a nodule or a "SOL" (space-occupying lesion), that is a separate question from fatty liver. Keep any follow-up scan you were asked to have, and take the report to a doctor.

Who this does not apply to

This guide is for adults with simple fatty liver. It does not cover children, pregnancy, hepatitis B or C, or people already diagnosed with cirrhosis or cancer. Each of those needs its own advice. Our Telugu glossary covers more liver words.

If you are unsure what the report means, call the helpline. Someone can explain it and help you decide who to see.

Questions we are asked

Common questions about fatty liver from Telugu families

Is fatty liver the same as liver cancer?

No. Fatty liver means extra fat in the liver. It is very common and most people with it never get liver cancer. Cancer risk rises only if fat leads to inflammation and then to serious scarring over many years, which is why doctors check for scarring rather than worrying about fat alone.

How do I explain fatty liver to my parents in Telugu?

You can say కాలేయంలో కొవ్వు చేరింది, meaning fat has collected in the liver. Explain that it is common, that it is not cancer, and that it improves with steady changes to food, activity, sugar control and alcohol. Bring them to the appointment so they can ask questions directly.

Can fatty liver go away?

Often, yes. Liver fat usually falls with gradual weight loss, regular activity, good sugar control and less or no alcohol. Even early scarring can improve. The changes need to be kept up, because fat can return if old habits come back. Your doctor can tell you what to aim for.

Does fatty liver cause pain on the right side?

Fatty liver usually causes no pain at all. Some people feel a dull heaviness under the right ribs. Pain there has many possible causes, including gallstones and muscle strain. If the pain is severe, comes with fever, or comes with yellow eyes, see a doctor the same day.

Which foods should we change at home?

Cut down on sweets, sugary tea, soft drinks and fried snacks. Eat smaller portions of rice or roti with more vegetables, dal and other protein. Cook with less oil. You do not need special products. Small steady changes that the whole family follows tend to last longest.

Is fatty liver passed down in families?

Families often share it because they share food, habits and a tendency to diabetes. Some genes also play a part. If one person has fatty liver, it is sensible for other adults in the house to have their sugar and weight checked. This is about protecting everyone, not blaming anyone.

Do we need regular scans for cancer?

Usually not for simple fatty liver. Regular surveillance scans are advised mainly for people with cirrhosis or advanced scarring. Your doctor will decide based on a stiffness scan and your other risks. If a lesion was seen, follow the advice you were given for that lesion.

Who should we see about fatty liver?

Start with a physician or a hepatologist, the doctor who specialises in the liver. If diabetes is involved, a diabetes doctor helps too. An oncologist is needed only if cancer is suspected. Call the CION helpline if you are unsure where to begin.
